Sign-up / Liability Form 

 

Parents who need to send in a sign-up form for their child can download it right from our site.

 

You may either:

 

Fax the form in to us at:

Scan and email it back to us,

 

Bring it to a school where your child attends and we have a staff member on site,

 

or parents may bring the form to a scheduled Y-MOT meeting - this will also give us an opportunity to meet you.
